The OQS service has been set up in response to demand from researchers who are interested in using electronic health questionnaires created for use in UK Biobank, in their own research projects. The service is administered by Oxford Population Health at the University of Oxford.

The questionnaires have been used to survey health and behaviours of UK Biobank participants, such as 24 hour dietary recall (Oxford WebQ) and cognitive function. The majority of the questionnaires have been developed in collaboration with researchers around the globe.

We have split the questionnaires into individual sections, or 'modules'. You can select the questionnaire modules or tests you'd like to administer to your study participants and develop your own electronic questionnaire, customising it with specific colours and a logo. Your study participants access the questionnaire via web links, and the resultant questionnaire data are stored on secure servers at Oxford Population Health, ready for download.
